Below is a checklist for parents and guardians to assist in the back to school transition.

**********ATTENTION ALL 5th Grade PARENTS************

The New Jersey Department of Health mandates additional immunization requirements for children after their 11th birthday. 


The following immunizations are required:
  •  Tdap booster (tetanus, diphtheria, acellular pertussis, given no earlier than the 11 th birthday. If a Tdap booster was received less than five years ago, the student will not be required to receive another dose until 5 years have lapsed. 
  • Meningococcal-containing vaccine, such as the meningococcal conjugate vaccine.

These immunizations are a requirement for 6th grade entry.  Students will not be permitted to enter 6th grade at the start of the 2020-2021 school year without these immunizations received and documentation provided to the school.  Please schedule your child’s annual physical now for a date right after his/her 11th birthday. If your child is already eleven years old, please have the immunizations done as soon as possible. Once the immunizations are received please provide documentation from the doctor’s office to the school nurse.

You may also fax the information to the number below.  These immunizations can also be given by the Gloucester County Department of Health for families without health insurance. They are located in Sewell and Paulsboro, NJ with set hours. Please visit their website: http://www.gloucestercountynj.gov/ or call (856) 218-4127 for further information.
